OBESITY
Disease Diagnosis:
The
year 2000 was the first in recorded history in which the percentage of
people worldwide who are obese exceeded the percentage of people who are
starving or malnourished. Americans are getting heavier just as the
rest of the world. Although obesity is common in all races, ages, and
both genders, African-American women are heavier than white or
Mexican-American women, Mexican-American men are heavier than white men,
and white men are heavier than African-American men. Men, as a group,
are more overweight than women in every age group but two, the 55-64 and
65-70 year old age groups.
Women however, still tend to be more obese
than men on average.

Hypertension
Diagnosis:
Hypertension is not
controlled. Patients today often make excuses for why things go wrong?
Often they fail to believe in scientific knowledge, or even if they
comprehend, they fail to implement simple changes in their lifestyles
which could save their lives once these changes become unconscious good
health habits. Much like obesity, it can be an insidious disease which
in many cases could have been avoided, controlled or delayed until the
golden years by sound unconscious life habits. Unlike Obesity, where
the patient and the world are readily able to acknowledge the condition
even unconsciously, folks often cannot perceive the effects of
hypertension and find the condition extremely difficult to put a handle
on, or believe they have it once proven so, which often requires much in
the way of patient education (Patient education is often a “priory for
success”). This absence of initial symptoms relative to even very high
blood pressures, often take the patient by surprise. As we see later in
this discussion under treatment, dependant upon the patient’s
presentation blood pressure levels, mild, moderate, severe, and so
forth, determines lifestyle changes combined with diet and exercise,
where a patient with a blood pressure of 230/110 absent symptoms would
need appropriate immediate medication. Many patients remain
inconvincible, and if patent education is absent early on, patient
abstinence from home and work lifestyle changes, adherence to exercise
and diet and taking necessary and often life saving medications remains
an obstacle for the patient’s true desire to “live” healthfully. Often a
part of patient education is explaining the catastrophic end organ
damage and circumstances or damage to the heart and kidneys as a direct
result of hypertension

wrong? We fail to believe in scientific knowledge, or even if we
comprehend, we fail to implement simple changes in our lifestyles.).

Diabetes Mellitus
Diagnosis:
As we have seen
with Obesity and Hypertension (Past
Two Articles for my Family Medicine Rotation 1a/1b and this article for my Jackson Park Rotation in Internal Medicine-attendant
Dr. Murthy, MD Hospital Director), it was the little things in life, made as
positive unconscious health habits, which when implemented can reverse
said conditions through a life commitment for health. As you will see,
both obesity and hypertension control are essential elements in
controlling diabetes. However Diabetes and its various types have
genetic or acquired links, receptor damage, cell damage necessary for
the production of prime hormones affecting the insulin and glucagon
worlds, which further complicate concepts of complete cure for even
moderate to severe disease.
Thus, Diabetes Mellitus is presently an
incurable yet in many cases more manageable then
previously (just the last decade) thought disease which is a major cause of premature death and
disability within the United States. As far back as 1997 and for that
year alone, direct medical cost and lost-productivity was measured at
$59 billions per year. And has been rising ever since.
For an alarming example,
Diabetes mellitus is on the
rise in the United States in adolescents with an elevated BMI.

DEPRESSION:
DIAGNOSIS:
Major Depressive Disorder or MDD, is one of the more commonly
encountered chronic relapsing psychiatric disorders, which causes
significant impairment to those afflicted. Depression is a feeling of
sadness and misery, usually accompanied by lowered self-esteem and
ranging from feelings of inadequacy and incompetence to a full-blown
delusional state.
Initial episodes often occur in adolescence although,
MDD can first develop late in life. Periods
of exacerbation of MDD are marked by anhedonia, feelings of failure,
inability to think positively about current life events or the future,
and suicidal thoughts. Both genetic and environmental influences
increase an individual’s risk of MDD. The periods of remission can last
for years, during which patients lead quite normal lives. Relapse has
been associated with stressful life events.
Within our nation, lifetime incidence of MDD is 20% in women and
12% in men. Prevalence is as high as 10% in patients observed in a
medical setting. Cultural influences on the presentation of depression
can be significant. The practitioner should be aware of differences in
the expression of psychological distress in patients from other
countries or cultures. Some cultural patterns are mentioned in the
Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition,
Text Revision (DSM-IV-TR); for example, MDD may be expressed
as fatigue, imbalance, or neurasthenia in patients of Asian origin. MDD
is a disorder with significant potential morbidity and mortality,
contributing as it does to suicide, medical illness, and disruption in
interpersonal relationships, substance abuse, and lost work time.

Diagnosis:
T he
fourth leading cause of death in the United States is Chronic
obstructive pulmonary disease (COPD), affecting approximately 16 million
Americans each year. About 100,000 deaths per year are directly
attributable to COPD, and it is a contributing factor to many more.
Almost 90 percent of COPD is associated with smoking tobacco affecting
at least 15 percent of those. Obviously, because more men smoke tobacco
then women, they are affected more often.
COPD occurs predominantly in
individuals older than 40 years.
COPD has been
recently defined by the National Heart, Lung, and Blood Institute and
the World Health Organization due to increasing concern. They formed
the Global Initiative for Chronic Obstructive Lung Disease (GOLD). In
their 2001 report, Global strategy for the Diagnosis, Management and
Prevention of COPD they define a new classification system for COPD that
places less emphasis on the distinction between chronic bronchitis and
emphysema than the previous statements. The GOD report was also
endorsed by the American Thoracic Society which defines COPD as
“a
disease state characterized by airflow limitation that is not fully
reversible. The airflow limitation is usually both progressive and
associated with an abnormal inflammatory response of the lungs to
noxious particles or gases.”
